The Tell SCOTMID UK survey will take about 5 minutes to complete and customers will need the following information from their receipt:
- Date of visit
- Time of visit
- Scottish Midland Co-operative store number
- Till number
- Transaction number
All of the above mention information can be found on the Scottish Midland Co-operative towards the top underneath the stores address. Lucky gift card winners will be selected at random on the last Monday of each month from all valid entries received before the last Saturday of each month. ALL Tell SCOTMID CONTEST entries received on the last Saturday of the month will be eligible for the next months drawing.

- The SCOTMID sweepstakes is open to all UK residents age 16 or older
- Please allow up to 60 days for the gift card to be delivered
- The £150 – £250 Scottish Midland Co-operative gift card must be accepted as offered and are non transferable (and cannot be turned in for cash money)
