When you submit a Nationwide rebate, you’ll need your official rebate form, plus a legible copy of your dated sales receipt (that includes date of purchase, products purchased including model number, the purchase price, and the name of the retailer), and the serial number for your device. Some Nationwide rebates are good for installation or delivery services. If you have one of those rebates, make sure that your receipt shows the date of delivery and installation along with the applicable charge.
If you only have one receipt, but you want to participate in multiple Natiowide rebate offers, you’ll just need to make a copy of your receipt. This is accepted in most cases.
