- Missouri and Illinois residents can sign up for the 5 Star SEP plan online from Essence Healthcare
The initial kit is free and customers will need to provide their name along with a street address (providing a phone number and email address are optional). Those who do provide a phone number and email address agree to have appointed Essence Healthcare sales representative call or email them in regards to plan questions and options. Any questions about the Star SEP free kit promotional offer can be directed to the Special Enrollment Hotline at 1-866-922-3065 (TTY: 711) Monday through Friday 8 a.m. to 8 p.m..

What is Essence Healthcare?
- Defined as a HMO plan with a Medicare contract
- Available to residents in the following counties of Missouri: Boone, Jefferson, St. Francois, St. Charles, St. Louis, or St. Louis City
- Available to residents in the following counties of Illinois: Madison, Monroe, or St. Clair
