Forever 21 Survey
- Provide feedback in relation to a visit to Forever 21 and receive a coupon for 10% off a future purchase (or if living in Mexico or Canada a sweepstakes drawing for a $250 Forever 21 gift card)
To take the Forever 21 survey customers will need the date of visit, transaction number, the amount of money spent at Forever 21, and the country in which the purchase was made (all of this information can be found on the Forever 21 customer receipt). Customers will also have the option to provide their email address in order to receive coupons from Forever 21 on a periodic basis. During the course of the Forever 21 Customer Sweepstakes Survey (for those living in Mexico and Canada) a total of 10 $250 gift cards will be given away during the promo.
